Job Description:
Full-Time Hours: Monday-Friday 8AM to 5PMSummary & ObjectiveThe Coordinator, Patient provides clerical support to patient care teams in relationship to patients.Essential Functions
  • Evaluates and ensures accuracy in the preparation of documentation.
  • Maintains files and other information in an organized manner to ensure follow-up and timely completion of assignments given by the Director, Clinical Care.
  • Maintains an excel file of all the visits as they are received in the centralized email account.
  • Tracks physician billing ensuring the billing sheets are approved by the Manager and keeping a record by Physician.
  • Tracks Contract bed visits and uploads notes into EMR system.
  • Tracks wound care visits and uploads notes into EMR system.
  • Maintains list of patients of all contract bed admissions in Hospitals for Dade and Broward.
  • Reports any variances between billing and EMR system to supervisor.
  • Maintains admission and certification tracking log in various hospitals.
  • Enters physician Weekly Assignments that have a note into the EMR system before month end.

  • Completes Data Entry using Excel.
  • Ensures effective communication with other Interdisciplinary Teams.
  • Respects and maximizes team effort through interactions with both clinical and support staff.
  • Establishes and maintains patient records.
  • Complies with all policies, local, state and federal laws and regulations.
  • Provides other duties of healthcare team members.
  • Performs other duties as assigned.
Supervisory Responsibility
  • No supervisory responsibility.

  • High School Diploma or equivalent education or equivalent combination of education and experience.
  • Must have knowledge of computer office/clinical software
  • Knowledge of Microsoft Excel and Outlook
  • Must be able to read, write and understand the English language
  • Ability and willingness to work a flexible Full-Time schedule that may include weekends and holidays
  • Must be able to provide own transportation to various locations in service areas, as required by duties
