General Summary:The Patient Flow Specialist works directly with the Capacity Management Command Center team to ensure patients admitted as observation and/or inpatient are placed in the right bed at the right time with the appropriate level of care. The Patient Flow Specialist reviews patient charts to determine if the level of care is appropriate and reports discrepancies to the attending physician. The Patient Flow Specialist reviews medical records for discharged patients to assist with navigating barriers to timely discharges.
Responsibilities:1. Provides hospital support for patient flow across Bayhealth supporting patient admissions, discharges, and documentation support.2. Accountable for command center functions that support patient flow and bed capacity. Assists with implementation and oversight of the command center operations including the development and implementation of operations, and process improvements. 3. Serves as a liaison between the command center and the campus leaders on all facets of transfer and bed management functions. This includes physician partners both internal and external to Bayhealth.4. Reviews patient status and anticipated bed availability/requirements to effectively management patient flow and throughput.5. Ensures maximum efficiency of resources related to optimal patient flow and staff assignment management.6. Provide just-in-time staff education and training on the policies, procedures, and practice standards applicable to Bayhealth. Reinforce policies and procedures and keep unit managers informed of issues. Accountable for actively engaging staff by supporting implementation of any new or changed processes.7. Assists in campus staffing oversight, in conjunction with, the command center team to ensure resource and productivity management, program development, and performance.8. Regularly provides assessments and feedback of processes and protocols and makes recommendations for process changes to enhance operations for effective direct admissions, transfers, information exchange and efficiency.9. Partners with leadership to ensure patient flow command center functions support development and growth of service lines. Partnerships include physicians, care managers, nursing, and external agencies.10. Assists in determining staffing needs based on acuity of each patient care unit.11. All other duties as assigned within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience:- Education: Bachelor Degree Nursing; Must be a graduate of an accredited nursing program/university
- Credential(s): Registered Nurse; Basic Life Support
- Experience: Required: Three years of nursing experiencePreferred: Experience in acute care setting.
