HOURLY RANGE: $23.539 - $30.022DISTINGUISHING FEATURES OF THE CLASS: The work involves performing a variety of clerical duties to facilitate continued patient care and support the Patient-Centered Medical Home (PCMH) model of care and outreach for patients at the Erie County Medical Center Corporation. The incumbent performs tasks such as collecting patient information, inputting patient information into an electronic health records system, distributing information and contacting patients and/or caregivers. Work is performed under the direct supervision of a higher-ranking employee with leeway for the exercise of independent judgment. Supervision is not a function of this position. Does related work as required.TYPICAL WORK ACTIVITIES:* Receives patient medical information and treatment goals and inputs into electronic health records (EHR) system;* Maintains and monitors the EHR to identify patients by risk factor, those requiring follow-up appointments and those not meeting treatment goals; distributes information to professional staff, medical personnel and related entities as requested;* Distributes correspondence such as appointment reminders and educational materials to patients and/or caregivers via various methods of communication; provides correspondence consistent with patients' and/or caregivers' specific language or medium;* Receives and responds to inquires from patients, caregivers and prospective patients in-person, via telephone or e-mail;* Distributes, collects and maintains patient satisfaction surveys; inputs information into separate electronic document or spreadsheet; distributes information to appropriate personnel;* Monitors EHR to identify patients who missed appointments, need to schedule an appointment, need filling or re-fill or medication and reminders to schedule follow-up appointments or tests; notifies patients and/or caregivers of same;* Serves as a liaison for patients and/or caregivers with various facilities to obtain test results; communicates same with patients and/or caregivers;* Assists patients in entering and monitoring personal health information in EHR;* Monitors EHR to identify discharged patients and contact them and/or their caregivers to assist in coordination of follow-up care;* Assists patients and/or caregivers in identifying primary care physicians or medical specialists for treatment;* Reviews patient medical charts and records for presence of advanced directives; communicates findings with appropriate personnel;* Develops statistics and patient data for inclusion in records, reports and other documentation utilizing computerized and electronic databases;* Reviews reports and other documents for completeness, accuracy and conformity with established procedures;* Types forms, records and other documents relating to patient activities;* Attends related training sessions and seminars;* Acts as a receptionist, takes telephone messages and provides routine information.FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Good knowledge of office terminology, procedures and equipment; good knowledge of hospital and medical terminology; good knowledge of guidelines, laws and regulations related to patient privacy and protected health information; good knowledge of clerical procedures; good knowledge of Patient-Centered Medical Home care model and principles; ability to understand and carry out oral and written instructions; ability to type from clear copy or rough draft in the input and/or retrieval of data; ability to compile and maintain information and records; ability to prepare reports; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications including electronic health records and Microsoft Word and Excel; ability to track information utilizing computerized and electronic databases; ability to work independently and make decisions in compliance with organizational standards; clerical aptitude; sound professional judgment; neatness; tact; confidentiality; courtesy; dependability; patience; capable of performing the essential functions of the position with or without reasonable accommodation.MINIMUM QUALIFICATIONS:Graduation from high school or possession of a high school equivalency diploma and three (3) years of clerical experience in a healthcare setting.NOTE: Verifiable part-time and/or volunteer experience will be pro-rated toward meeting full-time experience requirements.
