Position Summary:

Under the supervision of the Director of Surgical Services and the Director of Quality the Patient Navigator shall ensure patients find Joint Replacement surgery understandable (by providing education to improve knowledge, attitudes, and practices regarding their course of care), available (by knowing about services and linking clients to them), accessible (by removing structural and cultural barriers to services), affordable (by assisting clients to resources and programs for which they are eligible), appropriate (by offering culturally competent services and educating mainstream providers about the needs and cultures of their clients), and accountable (by participating in efforts to improve and expand services). Coordinates all aspects of the employee health program for WVMC hospital and clinics. Works collaboratively with all hospital departments, physicians and employees delivering services under the employee health program. Responsible for specific duties related to employee on boarding. Assists and collaborates with infection prevention activities. Implements the immunization program per guidelines and hospital policies and procedures. Facilitates the TB screening program for employees, volunteers and students per guidelines, policies and procedures.

Minimum Qualifications:

  • Bachelor of Science in Nursing, Masters preferred
  • Current Oregon Nursing, license.
  • Ideal candidate will have four years acute care experience.
  • Demonstrated success in impacting Nursing, practice through strong clinical leadership.
  • Must have the ability to work efficiently, responsibly, and independently. Excellent time management skills required.

Must possess strong interpersonal skills with a high sensitivity to the needs of others, including the ability to communicate effectively with staff, administrators, patients, families, physicians, volunteers, and other customers.

Essential Job Functions:

  • Demonstrates collaborative planning and teamwork with program development, implementation and evaluation of program developed in response to community needs including Pre-Operative Planning/Pre-Admission Testing Process, interdepartmental program staffing needs, joint surgery schedule, discharge planning patient representative and patient education
  • Provide Patient Education and Support

  • Provides support for Walk with Ease Class
  • Facilitates the provision of employment-related physicals and exams including pre-employment screenings and job related illnesses and injuries. Interview new hires for health history, fitness for duty, TB tests and drug Screening.
  • Surveillance of respiratory protection, tuberculosis, work-related injuries/illnesses/blood-borne pathogen exposures and immunizations of workforce including maintaining employee health records in EMR.
  • Documentation/record keeping, follow-up and case management of initial exam, work injuries and flu vaccine programs for employees, volunteers and physicians.
  • To keep informed of Workers Compensation, OSHA and CDC standards/guidelines. Facilitates light duty assignments and oversight.
  • Works with the Infection Prevention Coordinator to suggest preventive infection control measures and reports potential pathogenic disease processes to infection control with written reports submitted to the Infection Control Committee.
