Seeking a full-time employee as Patient Navigator I to work as part of a multidisciplinary team for the Primary Care Clinics. Employee is able to respond to referral sources from Manage Care Health Plans, Medical Groups, Medicare, and Hospitals. Duties include but are not limited to: Provide enhanced patient navigation services, track and Schedule All patients who are due for GAP, HEDIS, and Star Measures, Schedule Annual Wellness Exams. Along with other duties, employee is able to effectively manage a busy Call Center when needed.
Candidate will have at minimum two years working experience in Customer Service, Healthcare services, outreach, health promotion, Intake coordination, and/or similar or related fields. Candidate should have a strong working knowledge of Primary Care services. Candidates must have a willingness to participate in ongoing training in this field. Individual must be able to work efficiently independently and as part of a collaborative team. Excellent customer service, and organizational skills are a must. Bilingual Spanish is preferred.
