- Serves as a linkage between patients and primary care providers to improve the coordination of care.
- Collaborates with physicians, clinic staff and other interdisciplinary staff to identify care needs and connect patients with appropriate services.
- Coordinates care for patients who have gaps in care, or quality flags, to close these gaps by outreaching, engaging and scheduling patients.
- Coordinates patients scheduling, cancellations, and re-scheduling.
- Communicates with patients through telephone, mail, MyChart and text messages where appropriate, to increase patient attendance.
- Answers the telephone in a timely manner, politely and in a professional manner.
- Meets with patients, primary care and BH providers and other staff as needed to coordinate care.
- Encourages patient self-management with a focus on the achievement of care plan goals.
- Documents and maintains patient-specific records.
- Maintains accurate daily logs for statistical reporting purposes and for weekly reviews of productivity.
- Meets leadership when necessary to discuss various concerns involving the functions and performance of the Ambulatory Care Department.
- Completes mandatory hospital in-services via Healthstream.
- Log in to E-Mail, and any other necessary program daily.
- Performs any other duties as assigned.
Education Required - High school diploma, Bachelors preferredLicense/Certification/Registration(s) Required - NoneExperience/Skills Required:
- Minimum of two years' experience in the healthcare industry
- Advanced understanding of medical terminology and administration processes.
- Proficient in information management programs and MS Office.
- Outstanding communication and interpersonal abilities.
- Strong attention to detail with excellent organizational skills.
- Demonstrated proficiency in English and Spanish, preferred.
- Ability to work effectively in a team.
