Job Description:
The Patient Navigator functions as a point person for coordination of patient care to provide a smooth transition throughout the patient's cancer journey. Performs various administrative functions including, but not limited to, departmental reports, taking and documenting minutes and assisting withmeeting scheduling. The Patient Navigator conveys a positive image of the Cancer Institue while providing excellent customer service.1. Facilitate the elimination of barriers to treatment and supportive care.* Coordinate correspondence for patients referred to navigation and support.* Coordinate transportation for patients in need of assistance with this barrier.* Assess for benefits and determine if the patient has coverage for referred services.* Coordinate and interact with patient's families and caregivers as assigned.* Coordinate referral upon request for resources from a patient, family or caregiver.* Facilitate communication throughout the Cancer Institute.* Documentation of encounters in the EMR.2. Maintain monthly departmental reports.* Complete external foundation reports on a monthly basis.* Coordinate the monthly expenses related to transportation and submit report.* Obtain necessary information in the EMR for patient's utilizing grant monies.* Maintain monthly follow-up reports in compliance with Commission on Cancer standards.3. Customer Service/Office Duties* Answers the phone in a professional, courteous, and timely manner.* Efficiently route any calls/phone messages to appropriate staff in a timely manner.* Answer voicemails by the end of business day and complete requests promptly.Experience, Education, Training, Special Skills, and Licensure:* Experience: 3 years administrative experience preferably in healthcare* Education: High School Diploma or GED* Training: Proficient in Microsoft Word and Excel* Special Skills: Excellent written and verbal skills, Critical thinking skill, ability to think independently, anticipate needs and take initiative. High level of interpersonal skills to handle sensitive and confidential information.
