Position Highlights:
  • Position: Patient Navigator (MA or EMT)
  • Location: Grand Valley Oncology
  • Schedule: Five 8 hour shifts, Monday-Friday, Day Shift only, No weekends or holidays
Responsibilities:1. Provides patient care to assigned patients in the clinic per hospital standards and procedures.o Assesses patients, accurately obtains vital signs, such as pulse rate, temperature, blood pressure, weight, and height. Completes the MA Intake form and distress screening form. Accurately records information in the patients' charts.o Prepares treatment rooms for examination of patients.o Drapes patients with covering and positions instruments and equipment.o May give IM injections (vaccinations) to patients under the supervision of the Physician.o May performs venipuncture for the collection of laboratory specimens.o Assists RN with care coordination.2. Completes administrative functions as requested.o Answers the phone and QCLs within a timely manner.o Accurately enters data into computer to maintain office and patient records.o Maintains and performs quality controls on equipment as needed.o Performs chart prep for assigned physician ensuring the providers have all documentation required for patient visit in a timely mannero Charting requirements including health history, immunizations, allergies, drug samples, family history, social history, medical lists, etc.o Obtains Advance Beneficiary Notices as required.3. Assists with patient registries, EHR updates and training, process improvements, monitoring and reporting on meaningful use and other government required reporting.4. Scribing during physician visits when appropriate5. Ensure patients are scheduled and not lost to follow-up. Also responsible for updating patient schedules if/when appointments change.6. Responsible for following up immediately with oncology team if/when patients cancel, no show or reschedule appointments to ensure adequate coordination of care.7. Responsible for all appointment reminder calls8. This position is responsible for interacting with multiple teams where various stakeholders are involved including: oncology, radiology, prior authorization and scheduling.Requirements:
  • High school diploma or equivalent - required upon hire.
  • Associate's Degree in a health related filed - preferred.
  • Current BLS - required upon hire.
  • Current Medical Assistant Certification or EMT- required upon hire.
Compensation:
  • $21.00 - $29.40 per hour, depending on education and experience.
  • Discretionary bonuses, relocation expenses, merit increase, market adjustments, recognition bonuses, and other forms of discretionary compensation may be available.
