The Patient Navigator provides superior quality, competitive value and outstanding service by delivering exceptional support to the Case Management team, Emergency Room, hospital staff and patients. The Patient Navigator will report to the Case Manager Director and will assist inpatient and ED patients with special needs to ensure each health center meets these particular needs and requirements. Patient Navigators will focus on patients who lack a medical home. Responsibilities include: directing patients to different resources and information in the healthcare system, coordinating screenings and appointments, and encouraging patients. Under indirect supervision, the Patient Navigator will work closely with a specific patient population to improve health outcomes through navigation, post-discharge planning and advocacy through the social determinants of a health framework by making follow up appointments. May work with a multidisciplinary team including healthcare providers and a wide range of social services, as indicated by the patient, care team, and/or needs assessment(s).
