Sun River Health is currently seeking a full-time Patient Representative/Patient Navigator to join our team at our White Plains, NY location.A Patient Representative/Patient Navigator is responsible for providing patient centered care and facilitating patient access to services in a pleasant, knowledgeable efficient manner, communicating with a diverse population using well-developed interpersonal skills. This staff member will be responsible for all aspects of the registration and appointment process for patients. The staff member plays an integral role in the medical care team as the access point for patients to receive care. She/he will perform job duties in compliance with systems and job specific standards.Essential Duties and Responsibilities for a Patient Representative/Front Desk Receptionist:* Assist with daily reception activities and ensure that all processes needed for patient services are initiated.* Communicates effectively with team members and patients all aspects of service.* Responsible for telephone and mail contact with patients.* Facilitates patient care within different departments of Hudson River HealthCare and outside agencies.* Responsible for referring patients to appropriate agencies, with specific focus on referrals to specialty care and public health insurance programs, as needed and appropriate; track progress in eCW and alert providers.* Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance and application assistance.* Responsible for all necessary clinical information available to specialist for referral groups; tracks referral appointments and obtains consultant reports; obtains consent for release of information.* Assist patient in overcoming obstacles that would prevent them from receiving needed referrals.* Provide basic instruction/health education to clarify provider instructions, procedures and referral needs.* Responsible for the scheduling of appointments, fee collection, explanation of services and charges for patient services.* Verifies all insurance and sets fees based on the sliding scale fee.Minimum Education Requirement: High School diploma/GEDMinimum Work Related Experience: 2-4 yearsPreferred Language: Bilingual in both English and Spanish (orally and written)Sun River Health is a network of over 40 Federally Qualified Health Centers (FQHCs) providing primary, dental, pediatric, OB-GYN, and behavioral health care to over 245,000 patients annually.
