The Program Navigator is responsible for the overall care pathway and patient experience for the Jefferson Weight and Metabolic Health organization (Bariatric Medicine and Surgery). The Navigator is in direct contact with patients and is responsible for ensuring that patient needs and requirements are consistently met by the program. As a patient ambassador, the Navigator provides administrative, clerical, and service support to the clinics and virtual hub of the enterprise program.
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson
- Develops and maintains a deep expertise and understanding of the clinical programs and continuum of care for Jefferson Weight and Metabolic Health services
- Coordinates patient navigation through services within a clinical program by collaborating with multiple levels of office staff to ensure seamless patient care. Maintains communication with multidiscplinary clinicians to meet patient care objectives
- Works to provide exceptional patient care while maintaining a high volume of patients. Performs direct scheduling of patients and coordinates timely access for patients with a wide variety of clinical care providers
- Responsible for ensuring that all patients meet insurance and clinical requirements for bariatric surgery. Monitors and tracks progress for completing the necessary requirements for pre-operative bariatric surgery
- Maintains current knowledge of insurance requirements, approvals, and changes that impact the patient care pathway and thresholds. Assists with and supervises tracking f clinical requirements by each insurance provider
