Overview: The Patient Placement Coordinator is responsible for organizing patient placements for all patients in the organization's facilities. Working under close supervision, position helps optimize patient flow, making the processof bed placement and transfer easier for all parties involved. The schedule for this position is 3 - 12 hour night shifts-Wed, Thu, Friday 6p-6a /rotate every 6th weekend (Sat & Sun) 1p-9p.Talent Pool - Administrative & Clerical
Responsibilities:
- Coordinates bed assignments based on clinical status and physical condition to match people to appropriate locations, dependent on bed allocation criteria and bed availability.
- Helps manage the Emergency Department's track board for admissions to help keep a rapidly changing environment organized, noting bed availability and patients who may need a bed.
- Collaborates with nurses and physicians in and outside the organization to obtain pertinent information about patients which will help determine their placement.
- Works with patient placement Register Nurses (RNs) for clinical questions to gain the perspective of a clinician, synchronizing that point of view with the current status of bed availability.
- Communicates with Environmental Services to track and plan room cleaning operations, creating an efficient environment where new patients can go into clean rooms shortly after they are vacant.
- Performs other duties as assigned.
Qualifications:
