Job Description:

This leadership role will direct others toward process improvements that will support the reduction of medical/health care errors and other factors that contribute to unintended adverse patient outcomes. The PSO provides leadership for safety assessments, coordinates the activities of the High Reliability Program, educates other staff, leaders and providers on the system-based causes for medical error, consults with senior leaders, directors and medical staff, and communicates evidence-based guidelines regarding effective patient safety strategies organizationally.

ESSENTIAL JOB RESPONSIBILITIES

* Develops and utilizes surveillance systems to identify and mitigate potential patient harm.

* Analyzes data to identify trends.

* Assess near miss and actual errors through the occurrence system and facilitates follow up and analysis of errors with department leaders.

* Develops information to support and contributes to patient safety event classification process.

* Coordinates root cause, apparent cause and common cause analysis based on safety event classification.

* Coordinates activities of High Reliability and Patient Safety Program as required by State of Pennsylvania and accrediting body

* Develops and oversees processes to identify and analyze the process factors which contribute to healthcare related errors and adverse events based on knowledge of patient safety science.

* Facilitates team identification and implementation of action plans and process improvements.

* Investigates patient safety incidents organizationally. Facilitate root cause analysis of internal error reports.

* Reviews internal data and utilizes information from external reporting programs such as ISMP Medication Safety Alert, The Joint Commission Sentinel alerts, and ECRI Health Device Alerts.

* Recommends and facilitates change within the organization to improve patient safety based on identified risks.

* Designs and implements educational presentations that facilitate the understanding and implementation of patient safety standards within the organization.

* Supports and encourages error reporting throughout the organization by utilizing just culture principles.

* Develops and implements a recognition program aimed at improving patient safety.

* Provides reports to the Chief Medical Officer on the occurrence of known medical and healthcare incidents, identified near misses, and dangerous conditions organizationally as well as actions taken, either proactively, or based on occurrences as well as barriers to implementation of safety programs.

* Develops reports to clearly communicate the results of analysis to staff, hospital leaders, senior leadership and providers.

* Maintains awareness of legislative updates related to patient safety.

Minimum Education Required: A Bachelor's degree in Nursing or other healthcare related field; Master's degree preferred.

Minimum Experience Required: Minimum of three (3) years clinical experience in healthcare.

Licensure/Certification Preferred: Certified Professional in Patient Safety or must obtain within two (2) years of eligibility.
