All while understanding that together we are even better.Schedule: Full-time Monday-Friday; Days 8-hour shifts; No weekends/holidays; May travel to other hospitals as neededLocation: UChicago Medicine AdventHealth Hinsdale Hospital, 120 N Oak Street, Hinsdale, ILThe role you'll contribute:The Patient Safety Specialist provides leadership and direction in developing a culture of safety and in identifying opportunities and initiatives to enhance patient safety. They promote and enhance patient safety through oversight of the facility-wide patient safety program. The Patient Safety Specialist is primarily responsible for proactive patient safety surveillance, overseeing patient safety initiatives, patient safety event investigation and/or adverse event follow-up improvements. This role will work to improve the delivery of evidence-based care and reduction in preventable harm by focusing on principles of high reliability.The value you'll bring to the team:* Promote an organization-wide approach to increasing patient safety and reducing error within the hospital settings.* Monitor and track the safety event reporting system and maintains confidential documentation on investigations.* Lead, support and facilitate the use of tools and implementation of systems/processes that identify patient safety risks, promote a culture of safety, reduce patient harm, and improve high reliability healthcare.* Facilitate and participate in causal analyses. Develop RCA and ACA action plans in collaboration with clinical and operational leadership. Identify actions, measures and interventions that address root and apparent causes. Monitor and support timely and effective implementation.* Facilitate meetings, initiate task forces or work groups to directly remediate and resolve quality of care, patient safety or clinical systems issues.* Facilitate weekly Safety Event Review Team (SERT) meetings.* Analyze patient care information from multiple data sources and reports on results.* Coordinate serious reportable events and sentinel event communication and follow-up with appropriate internal stakeholders and external agencies.* Utilize alerts and best practices (e.g. Sentinel Event Alerts) to perform gap assessments and implement strong actions that will alleviate identified gaps.* Collaborate and oversee the hospital-based Patient Safety Coach program. Recruits and trains department patient safety coaches. Coordinates monthly safety coach meetings.* Share patient safety lessons learned and safety alerts across UCM AdventHealth Great Lakes Region and coordinate collaborative patient safety efforts. Coordinate communication with bedside staff and other relevant stakeholders regarding improvements related to adverse events.* Assist in the development and delivery of patient safety, high reliability, and culture of safety education. Mentor, train, and orient staff in patient safety.* Oversee and maintains applicable patient safety dashboard(s), developing and tracking patient safety related goals and disseminating information to relevant stakeholders.* Maintain collaborative relationships with other teams/departments including, but not limited to, Risk Management, Quality and Patient Safety, Accreditation and Compliance, Medical Staff and Nursing.* Participate in the development, review, and revision of hospital policies and procedures that affect patient safety.* Participate in relevant committee meetings (e.g., Quality and Patient Safety Committee) to offer support and guidance. Provides constructive feedback, as needed.* Round in patient care areas to promote event reporting and to facilitate communication of lessons learned from adverse patient care event investigations and analysis.* Other quality and patient safety duties, as assigned.QualificationsThe expertise and experiences you'll need to succeed:_EDUCATION AND EXPERIENCE REQUIRED:_* Bachelor's degree from an accredited school/college of nursing, allied health, or healthcare related field.* Minimum of 5 years' experience in hospital/acute care (2 years must be within the past 3 years)_EDUCATION AND EXPERIENCE PREFERRED:_* Master's degree in healthcare-related field preferred.* Experience facilitating root cause analysis, failure mode effect analysis, and error investigations.* Experience in quality, patient safety or risk managementLICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:* Current Illinois state license in field of study as required based on degree (e.g., Registered Nurse in State of IL)_LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:_* Patient safety related certification (CPPS, CPHQ, CPHRM, etc.)KNOWLEDGE AND SKILLS REQUIRED:* Ability to work with an electronic health record.* Requires strong interpersonal and communication skills, both oral and written.* Ability to work well with senior management, directors, medical staff, and employees at all levels of the organization and externally.* Requires excellent computer skills, including advanced use of Microsoft Word, Excel, and PowerPoint._KNOWLEDGE AND SKILLS PREFERRED:_* Working knowledge of patient safety science and high reliability principles.This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
