We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.The Patient Service Representative is responsible for greeting guests to the health center, completing the patient intake process, scheduling patient appointments, and monitoring waiting room activity.RequirementsEducation: High School Diploma or equivalent required. Experience: Basic computer skills, flexibility with scheduling for occasional Saturday or weekday evening hours as needed, and able to travel between sites within Manchester.
