Job Description:
A Personal Care Assistant is an individual who provides support services to assist clients with personal, physical mobility and therapeutic care needs, as established by a rehabilitation health practitioner, social worker or other health care professional. The Personal Care Assistant may work in a variety of settings, to include medical facilities, rehabilitation centers or in-home residential care. Essential Duties and Responsibilities:
  • Provides services to persons who need help with day-to-day activities
  • Demonstrates competence in performing the necessary skills based on the patient's plan of care
  • Provides health care tasks, personal hygiene services, and other related support services essential to the client's health and safety
  • Assists with transfers and ambulation
  • Performs incidental household services that are an integral part of a personal care plan (if applicable)
  • Provides companionship by reading, listening and talking with client
  • Assists client in the development of independent skills through self-care activities such as toileting, clothing routines, and personal hygiene
  • Implements behavior management techniques in accordance with the directions given by the supervisor/healthcare practitioner; including reinforcing appropriate behaviors in accordance with plans and discouraging negative behavior
  • Reports changes in patient/client status
  • Adheres to the Health Insurance Portability and Accountability Act (HIPPA) privacy policies and procedures
  • Performs other duties as assigned/necessary
Minimum Requirements:
  • State licensure or certification if required by state and/or contract
  • One (1) year relevant patient/client care experience preferred
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB Questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be required
  • Computer proficiency required
  • Must be at least 18 years of age
