Our client is a leading community health organization dedicated to providing accessible, high-quality care to diverse populations. We are seeking a skilled and compassionate Family Nurse Practitioner (FNP) to join our client's outpatient team and deliver comprehensive care to patients of all ages.The Family Nurse Practitioner will provide primary and preventative care services to individuals and families in an outpatient community health setting. This role involves diagnosing and managing a range of acute and chronic conditions, promoting wellness, and addressing the unique healthcare needs of underserved populations.*Key Responsibilities:** Deliver patient-centered, evidence-based care to patients of all ages, from pediatrics to geriatrics.* Perform comprehensive health assessments, including physical exams, routine screenings, and wellness visits.* Diagnose and manage acute and chronic illnesses, injuries, and conditions.* Prescribe medications, treatments, and therapies in accordance with state and federal regulations.* Educate patients and families on disease prevention, health maintenance, and management of chronic conditions.* Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and other healthcare providers, to coordinate care.* Refer patients to specialists or community resources as needed.* Maintain accurate, timely, and compliant patient records using electronic medical record systems.* Participate in community health initiatives, outreach programs, and quality improvement projects.*Requirements:** Master's or Doctorate degree in Nursing from an accredited program with a focus on Family Practice.* Current and unrestricted Nurse Practitioner license in [state].* National certification as a Family Nurse Practitioner (FNP-BC or FNP-C).* Current CPR/BLS certification.* Minimum of 1-2 years of experience as a Family Nurse Practitioner preferred.* Experience in a community health or outpatient setting is highly desirable.* Strong clinical and diagnostic skills across the lifespan.* Excellent communication, organizational, and interpersonal skills.* Ability to work effectively in a culturally diverse environment and address social determinants of health._Please note- The annual base salary range is $140k to $160k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
