Job Description:
JOB DEFINITION:The role of the Recovery Advocate Specialist is to provide support, advocacy and linkage services to individuals and/or their affected families when an individual is brought to hospital emergency departments due to heroin or opiate prescription use and/or overdose, and/or due to other substance use. Must have a valid driver's license.EDUCATIONAL REQUIREMENTS:High School or Equivalent. Associate, Undergraduate or Master's level Degree a plus. Knowledge of substance use disorders prevention, treatment and referrals a must. CRPA or CRPA-Provisional also a plus.EXPERIENCE REQUIRED:Relevant life experience required which includes personal experience living with a disability, history of substance use, or social/emotional challenge, and willingness to draw upon this experience to serve as a role model to support the recovery goals of individuals served. Must have a valid driver's license.DUTIES AND RESPONSIBILITIES:
  • Engage people on the street and in hospital emergency rooms, developing brief person-centered service plans, providing referrals and linkages to needed services. Provide support, encouragement and guidance in linking persons to appropriate services.
  • Provides outreach, advocacy, mentoring and recovery support services to those seeking or sustaining recovery. Provide strategies for building recovery capital appropriate to the goals of the individuals they work with.
  • Working collaboratively with Hospital SBIRT Health Coaches, will meet with individuals and/or family/significant others at local hospitals ED's or agreed upon locations if discharged to assess their needs and connect them to treatment and other support services across Nassau and/or Suffolk Counties.
  • Remain in an active role and provide follow-up on the individual and/or families @ 30, 60-, 90-, 120-, and 180-days' post discharge from ED.
  • Collect any necessary data to assist in reporting to funding sources and/or hospital system.
  • Act as liaison between the Program and the communities it serves.
  • Represent the Program in a professional knowledgeable manner.
  • Assist in the promotion of the Program through information and education about its services to the Communities we serve.
  • Provide direct outreach to courts, attorneys, hospitals, probation, parole, DSS, CPS, DDP, MDs, Schools and various other community groups with direct referral back to the Agency and its Program(s) if necessary.
  • Represent the Program at various community events, such as Narcan Trainings, Fairs, Forums, etc. as possible.
  • Assist in the creation of flyers and promotion information with and under the direction of the Program Director(s).
  • Document and report on contacts made in the community with the Program Director(s) and hospital reporting system. Documentation should include name of contact, agency if applicable, address and phone number and any other required information.
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress.

  • Identify and implement quality improvement trainings and activities as part of professional development goals
  • Encourage quality improvement processes and provide consistent resources for QI and professional development

  • Other duties and assignments as requested by the Team Leader.
