The Peer Coordinator will have lived experience with homelessness, incarceration and or behavioral health needs. The Peer Coordinator is responsible for conducting outreach to community-based organizations to assist with the recruitment of homeless persons, for emergency/transitional housing placement at the Fortune Academy located in Harlem. The Peer Coordinator will work collaboratively externally and internally within The Fortune Society network of programs. These activities include, but are not limited to: screening, assessing, counseling and linking homeless, formerly incarcerated residents with histories of mental illness, substance use and/or HIV/AIDS to needed emergency/transitional housing. The Peer Coordinator will be under the direct supervision of the Director for Social Services. - Document client contact and services in agency's proprietary database (CaseWorthy) and external funder databases.
- Effectively engage potential participants in the community.
- Assess need for emergency/transitional housing and conduct intakes
- Maintain accurate records in a timely and orderly fashion.
- Maintain relationships and liaison with treatment providers and community based organizations in the community
- Orientate new participants to the Fortune Academy program and facility
- Provide linkage to Services (Benefits, Medical, Substance Abuse, Mental Health)
- Escort participants to scheduled appointments as needed
- Facilitation of groups with participants (Anger Management, Life Skills, Housing Readiness, etc,)
- Maintaining a substance free environment by performing random toxicology testing of participants as needed
- Provide crisis intervention and mediation as needed
- Schedule and facilitate positive social activities with participants
- Other tasks and responsibilities as the job may require.
QualificationsQUALIFICATIONS:
- High school diploma or GED required; some college preferred.
- At least 2 years of experience working with homeless population, HIV/AIDS, substance abuse and/or legal system issues.
- Certified Peer and or Recovery Coach Certification preferred.
- Experience using Microsoft Office programs and database case management systems.
- Able to encourage disenfranchised populations to seek and obtain needed social services.
- Strong follow-up, time management and organizational skills.
- Ability to work well on your own and be self-directed, but also keep supervisor updated and share information and collaborate with a team.
- Strong verbal and written communication skills.
- A valid driver's license preferred.
