Job Type Part-timeDescriptionHVCS a Division of Cornerstone Family Healthcare is actively recruiting for a Peer Navigator to join our growing team in Monticello.RATE OF PAY/SALARY: $18.00 per hourWORK LOCATION(S): Monticello, NYSTATUS: Part-TimeSchedule:14 hour per week JOB SUMMARY:Under the supervision of the Program Supervisor, the Peer Navigator will work in tandem with the agency's harm reduction and outreach programs to provide services to people who use/inject drugs (PWUD/PWID) within communities in Dutchess, Orange, Ulster, and Sullivan Counties. One Peer position will cover Orange and Sullivan counties and one peer position will cover Dutchess and Ulster counties.Key Competencies :
- Participate in outreach to PWUD/PWID at the community level as well as within shelters, soup kitchens and other community-based organizations to publicize program services and recruit clients.
- Participate in outreach to community partners and work with harm reduction programs staff to increase the priority population's awareness and understanding of overdose and other health related issues.
- Participate in providing harm reduction education, counseling, and supplies.
- Provide referrals to clients for holistic medical, behavioral health, testing, drug treatment and other support services as needed.
- Complete such trainings as assigned by the Program Supervisor.
Description of Duties:
- Observe and abide by HIPAA and the HIV Confidentiality Law.
- Use personal contacts, social media and other networking strategies to engage the priority population.
- Assist the program in promoting and implementing program services with the community and priority population.
- Pass on to Program Supervisor, any referrals for HIV/STI/HCV testing and counseling, PrEP/PEP, drug treatment, medical, behavioral health, social services, as well as other supportive services.
- Maintain an awareness of trends impacting the priority population to provide to the Program Supervisor to adjust programming as needed.
- Complete the mandated continuing education trainings as selected and scheduled by the Program Supervisor.
- Assist in program coverage when required including street outreach, PDSE, and mobile unit.
- Attend and participate in monthly department, All Staff and other required meetings.
- Be familiar with Cornerstone policies and procedures and the Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other related duties as assigned.
- While performing the duties of this job, the employee is exposed to climate and noise conditions prevalent at the time as well as virus, disease and infection relative to patient contact and specific job duties. The noise level in the work environment is usually minimal, and includes but is not limited to voices, office machines and cleaning equipment.
Requirements
- A High School Diploma with experience in peer education, harm reduction, HIV/HCV/STI and/or disease prevention.
- Familiarity with the priority population and the geographic area assigned. Commitment to undergo training about HIV/HCVSTI, harm reduction, social support services, and community resources.
- Must have a valid, unrestricted driver's license and independent means of transportation for frequent travel to locations throughout the assigned county as well as to the offices of Cornerstone.
