Initiates contact with individuals in residential treatment to introduce Recovery Support Services and develop Recovery Support Plan within 30 days of admission. Reviews plans within required timelines and revises plan as needed. 2. Attends clinical staffings and reviews clinical record to determine appropriate recovery support plan. Revises and updates plan weekly as treatment progresses. Presents updates at clinical staffings.3. Facilitates structured and organized psychoeducation and recovery-oriented support groups a minimum of one (1) hour per week. 4. Provides individual and family sessions as needed for individuals who have completed an A&D treatment program. 5. Provides interim services (i.e., individual, group, family support) for individuals seeking services and are on a waiting list. 6. Provides linkage and referral to community resources, as well as, necessary supports that will work towards improving outcomes for individuals engaged in Recovery Support Services as identified on the ISP and Recovery Support Plan. 7. Monitors individuals who participate in Recovery Support Services and reside in Caprice Sober Living and/or community housing through HUD rapid-rehousing funds. 8. Provides didactic-educational groups in the residential treatment program addressing recovery issues as assigned by the Director of A&D Services or his/her designee.9. Provides peer-to-peer mentoring and coaching. Initiates follow-up contacts, via phone or face-to-face, with former clients to determine sobriety status and current needs. Maintains contact (or documented efforts to contact) for a period of 12 months following discharge with emphasis placed on the first sex (6) months of Recovery Support Services as defined by the DMH. 10. Offers supports to individuals who relapse to help reduce the negative effects, and if necessary, refer to appropriate level of treatment. 11. Assists individuals by modeling effective coping techniques and self-help strategies based on prior experience, supporting individuals in advocating for themselves to obtain effective services, and helping them to monitor their progress.12. Establishes and maintains referral relationships with community agencies including law enforcement, courts, the recovering community, area health care professionals, DHS, schools, employers, and churches.13. Creates relationships with other Recovery-Oriented organizations and groups to help individuals integrate back into their community, develop a support system, and acquire a sense of belonging.14. Develops an annual plan for conducing community outreach activities to educate and discuss addiction, mental health, and recovery. Conducts a minimum of twelve (12) community activities per year and provided in each county in our catchment area. 15. Provides a pre-admission Alcohol and Drug assessment when clinically indicated to determine an appropriate referral recommendation.16. Maintains client confidentiality at all times.Documentation and Quality Improvement:17. Maintains case record documentation in compliance with DMH standards, including completing progress notes when indicated. 18. Completes service logs/documentation in compliance with agency policy. 19. Develops Recovery Support Plans and reviews/updates in compliance with DMH standards and agency policy. 20. Maintains an active and ongoing contact /referral list including agency or individual name, address, phone number, contact person, and record of contact dates and times.Interpersonal and Leadership Skills:21. Shares their unique insight into A&D concerns and mental illness with non-consumer staff and what makes recovery possible.22. Serves as a role model and emphasize the value of each individual's recovery experience.23. Responds to clients needs in an appropriate and timely manner.24. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.Support Duties:25. Transports clients as assigned in a safe and timely manner. If using a CCS vehicle, signs vehicle in and out per agency policy. Completes inspection and documentation prior to driving. Ensures that vehicles remains clean and in good repair.26. Identifies and reports to the Director of A&D Services or his/her designee in a timely manner any safety concern in the work environment.27. Completes additional job duties assigned by supervisor.Key ResponsibilitiesESSENTIAL QUALIFICATIONS/SKILLS/EXPERIENCE:Ability to be certified and complete DMH Certified Peer Support Specialist programDemonstrate a minimum of six (6) months in self-directed recovery.A current or former consumer or first degree family member of an individual who has received treatment for and self-identifies as a consumer or former mental health consumer. Requires excellent organizational and interpersonal skills.Requires satisfactory completion of CPR prior to reporting to the work site. Requires satisfactory completion of CPI within the first 30 days of employment.Requires ability to be covered under company insurance to drive a company vehicle for client transportation.Requires valid driver's license and current auto liability insurance.Requires ability to transport clients in a private vehicle when necessary.Requires completion of annual training in compliance with DMH standards.Requires thorough knowledge of the 12-Step Model of Recovery.PREFERRED QUALIFICATIONS/SKILLS/EXPERIENCE:Previous experience in an A & D treatment setting strongly preferred.Minimum EducationHSMinimum RequirementsHS degree and peer support certification required, Bachelor's degree in behavioral science and addiction certification preferred.Licensure/CertificationPeer support certification or ability to obtain.Applications Close Date
