Job DetailsJob Location213 West 35th St SCIF Clinic - New York, NYPosition TypeFull TimeEducation LevelHigh SchoolSalary Range$45,000.00 - $50,000.00 SalaryDescriptionJOB SCOPE:The CCBHC Peer Specialist provides direct services geared towards assisting individuals with histories of medical, substance use, and mental health needs, engage in treatment and promote wellness. The Peer Specialist provides support services on and off-site to engage individuals on a continuum of care to help them improve wellness and functioning in the community. Services include individual and groups meetings to engage and assist individuals identify needed services. The Peer Specialist must be skilled in engagement, advocacy, and recognizing crisis situations. The position is required to provide services both in the office and in the community, to ensure that individual needs are met. The Peer Specialist will liaison with community agencies, health care providers and others on behalf of individuals to facilitate access to services to improve their ability to function in the community and increase wellness self-management. This position operates as part of a multi-disciplinary CCBHC team and requires collaboration with the clinical treatment team, and external entities. The CCBHC Peer Specialist is responsible for participating in mobile crisis team, with rotating on-call coverage to respond to community-based crises outside of clinic operating hours. ESSENTIAL FUNCTIONS:
  • Conduct in-person outreach to individuals to promote engagement with treatment.
  • Provide support in times of crisis such as after hospitalizations.
  • Assist individuals in completing wellness recovery plans as needed.
  • Provide outreach to individuals who are at risk of being lost to care to engage in treatment.
  • Provide resources and connect individuals to appropriate and available support services.
  • Accompany individuals to appointments and conduct home and community visits as needed.
  • Promote and explore positive social activities and community integration.
  • Facilitate groups including ADL's, recovery peer support, wellness groups and others.
  • Participate in outreach events as assigned to represent the agency.
  • Document all services and outreach efforts via progress notes in the EHR within 24hours.
  • Collaborate with CCBHC staff regarding the coordination of care and services.
  • Maintain positive working relationships with all agency staff, individuals and visitors.
  • Responsible for emergency on call after hours coverage for the Crisis Team.
  • Performs other duties, consistent with the goals and objectives of the program, as directed by CCBHC or PCMH Administration.
QualificationsSKILLS AND KNOWLEDGE:
  • Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
  • Must be able to use outlook, word, and excel.
  • Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
EDUCATION AND EXPERIENCE:
  • Full CRPA or CPS required (Certified Recovery Peer Advocate, Certified Peer Specialist)
  • Experience in behavioral health and social service settings.
  • Experience working with mental health and substance use disorder populations.
  • Experience with facilitating groups
  • Ability to work collaboratively as part of a team
  • Must be outcome oriented in a fast-paced environment with deadlines.
