In this role, the successful candidate Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, identification of expected outcomes, planning, implementation and evaluation. Provides competent, safe and compassionate care based on established NYU Langone Health Patient Care and Nursing Standards. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act.Job Responsibilities:
    • Demonstrates a solid foundation and understanding of organizational goals as outlined within the quality and safety performance improvement plan (QSPIP) and nurse sensitive quality indicators (Include unit based quality measures e.g. HAPU, CAUTI, CLABSI, Falls).
    • Collects, assesses and documents patient healthcare data.
    • Involves patient/family and Interdisciplinary team to identify a plan of care and outcomes that are realistic and measurable.
    • Prioritizes care based upon data collected.
    • Documents assessment and reassessment in medical records.
    • Implements interventions in the coordination of patient care in a safe, timely and appropriate manner and applies escalation process.
    • Considers needs and behaviors of specific patient age groups and cultural groups in all patient care activities.
    • Documents interventions per hospital standard.
    • Educates the patient/family regarding health status and established plan of care.
    • Determines learning needs of the patient/family. Implements an educational plan. Collaborates with the interdisciplinary team to modify expected outcomes.
    • Documents learning outcomes of patient/family and revises education plan. (Include utilization of EHR education tools)
    • Evaluates the patient's progress towards expected outcomes. Involves the patient/family and interdisciplinary team in the evaluation process, recognizes changes in patient condition, reports documents and evaluates the effectiveness of the intervention(s).
    • Participates in and contributes to in unit activities to provide patient care in a healing, humane and caring environment.
    • Demonstrates understanding of and contributes to unit throughput efficiencies and metrics. (Include unit throughput measures e.g. DBN, door to triage, assign to occupy, first case starts, room turnover)
    • Complies with unit based and organizations professional standards, New York State Nurse Practice Act and regulations of relevant outside agencies, e.g., NY State Department of Health, The Joint Commission, regulatory requirements etc.
    • Exhibits appropriate judgement in delegated activities.
    • Demonstrates ability to differentiate those activities which are in scope of professional nursing practice, and those that may be delegated to ancillary staff.
    • Complies with annual education and mandatory/regulatory requirements (Include learning transcript and supportive documentation).
    • Serves as a support and resource for nursing staff and students.
    • Engages in self-assessment and peer review; utilizes resources to meet individual learning need and promote professional development in clinical practice.
    • Adheres to patient care and nursing standards in care delivery, and uses clinical practice guidelines (CPG).
    • Attends unit practice councils (UPC) and utilizes the practice council structure to critically appraise and influence practice (Include supportive evidence).
Additional Position Specific Responsibilities:- Introduces patients, family members/caregivers, providers, and staff to holistic nursing theory, principles of integrative health and mind-body modalities to promote healing, reduce pain, encourage relaxation, and provide emotional support- Empowers patients and family members/caregivers to be active participants in holistic interventions and empowers patients to be actively engaged in their own healing. Collaborates with patients in designing holistic care plans.- Explains treatment, sets expectations with patient. - Recognizes changes in patient condition and responds to variations in patients progress by modifying the interventions.- Reviews chart to determine consult indication, inclusion & exclusion criteria, and contraindications to services. Uses Nursing assessment skills to determine consult appropriateness, takes appropriate steps to collaborate with proxy and team if patient is unable to consent.- Optimizes comfort level wherever possible. Coordinates and directs activities to provide patient care in a healing, humane and caring environment.- Observes and assesses patient/family responses during and after intervention. Documents holistic assessment, intervention, education, response, and any follow up. Communicates with primary clinical team as needed. Escalates care concerns, advocates for patient.- Provides education to the NYULH community about Department of Integrative Health services and scope of practice.- Serves as a support, resource and preceptor for nursing staff, students, and medical residents.- Comfortable in application of trauma-informed care with cultural sensitivity across the lifespan. Demonstrates ability to flex between patient/family care focus to involvement in research protocols, providing educational offerings, making presentations, and assisting in content creation/program development.- Engages in self-assessment and peer review; utilizes resources to meet individual learning needs and promotes professional development in clinical practice.- Contributes to and utilizes the unit practice council (UPC) structure to discuss clinical experiences and influence practice. - Comfortable in application of trauma-informed care with cultural sensitivity across the lifespan. Demonstrates ability to flex between patient/family care focus to involvement in research protocols, providing educational offerings, making presentations, and assisting in content creation/program development.- Ability to clearly and effectively communicate with diverse populations, including providers and other healthcare staff, patients and family/visitors, administrators, and members of the NYU Langone Health Community amongst different platforms.- Communicates with clinical staff when a patient is in acute distress, engage interdisciplinary team and/or administration as needed.- Considers needs and behaviors of specific patient age groups and cultural groups in all patient care activities; attends trainings as offered.Minimum Qualifications:To qualify you must have a Current Registered Professional Nurse in New York, Bachelor's degree in Nursing required, Basic Life Support Certification through the American Heart Association, Fluent written and spoken English language skills.. Required Licenses: Registered Nurse License-NYS, Basic Life Support CertPreferred Qualifications:- Completion of accredited program in holistic nursing/CAM/Integrative health and wellness.- Board Certification in Holistic Nursing- 2 years experience providing at least 2 Holistic Modalities such as: Reiki, Meditation, Reflexology, Nurse Coaching, Acupressure, Mindfulness, M-technique.- Certification in at least 1 of the above modalities.- Experience as charge nurse.Qualified candidates must be able to effectively communicate with all levels of the organization.NYU Langone Health provides its staff with far more than just a place to work.
