What We Are Looking For Job Title: Per Diem Residence Registered Nurse - (Days) Schedule: 8:30 am - 4:30 pm (which can include weekends in the community) Essential Job Functions/Responsibilities
  • Establishes priorities and organizes work and time appropriately.
  • Anticipates problems and modifies plans and procedures to provide solutions.
  • Assesses and assigns work assignments to ensure complete utilization of staff, and quality patient/family care.
  • Ensures medications and supplies are available for staff to deliver quality patient care.
  • Participates in the evaluation and review of services being provided to each patient and family. Collaborates with interdisciplinary team and other agencies/professionals as needed to facilitate care.
  • Provides direct patient care as needed on an emergency basis.
  • Performs accurate initial assessment with appropriate follow though and timely, accurate admission documentation.
  • Generates initial care plan and performs on-going assessment of patient and family needs, updating the nursing care plan accordingly.
  • Performs accurate on-going symptoms assessment with appropriate management and follow though, inclusive of physical, psychosocial and spiritual needs.
  • Maintains proficiency working with technical equipment. (Enteral feeding pumps, suction machines, IV medication pumps, hydration pumps, etc.)
  • Maintains proficiency working with IV access devices including IV medication administration, IV maintenance, and documentation of same.
  • Administers medication as ordered according to Hospice Residence policy and documents accurately.
  • Ensures documentation in the patient's medical record is accurate and complete, and the contents reflect timeliness, accuracy, and a clear clinical picture of the patient.
  • Co-ordinates patient care services rendered in the Hospice Residence while maintaining standards of care consistent with the Hospice philosophy.
  • Communicates effectively with the physicians.
  • Co-ordinates patient transfer between the Hospice Residence and other departments/facilities.
  • Participates in disciplinary meetings with employees per policy.
  • Functions in a leadership role for the staff of that shift, assisting them in resolving issues and bringing unresolved concerns to the director.
  • Handles patient/family and personal grievances and documents per agency policy, referring incidents to the Director.
  • Interfaces professionally with physicians, vendors, other agencies/facilities, and other departments within Hospice.
  • Participates in family conferences when appropriate.
  • Assists in evaluation of Hospice Residence staff as per policy.
  • Assists in evaluation and implementation of orientation program for new Hospice Residence employees.

  • Candidates must have a NYS RN Licensure with 1-2 years of recent experience employed as an RN in hospital setting, home care, or skilled nursing facility.
  • Graduate of an accredited school of Nursing and currently licensed to practice in New York State.
  • Applicant must demonstrate basic knowledge of nursing theory and practice, knowledge of appropriate technology, knowledge of practice standards for area of responsibility, sound clinical skills, sound standards of practice, and positive communication skills.
  • Previous hospice/home care experience preferred.
  • Hospice is an equal opportunity employer.

Job Limitations Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment.
