JOB DESCRIPTION:

Summary/Objective

Boston Senior Home Care's Home Care Management and/or MassHealth Programs include the following programs: Home Care Management (HC), Coordination of Care (CAE) Adult Foster Care/Shared Living Program (AFC), Group Adult Foster Care/Home Power (GAFC), and Personal Care Attendant (PCA). The Programs are clinically based programs funded by EOEA and/or MassHealth and administered by Boston Senior Home Care with the goal of keeping elderly and disabled individuals in the community.

PDRNs will be called upon on a per needed basis for clinical assessments, initial assessments and re-assessments, not to exceed 15 hour per week which equals 780 hour per year, to help determine eligibility for the programs/services and develop a comprehensive service plan based on his/her assessment. This position is a critical part of the interdisciplinary case management team in assessing client care needs and appropriate service plans for new and ongoing clients. The PDRN may also carry an ongoing caseload consisting of bi-monthly and/or monthly home visits to assess clients' continued program eligibility and compliance, monitoring of personal care tasks, and psychosocial needs. The PDRN works collaboratively with informal and formal supports, vendors and other community agencies to support clients and their independence.

Essential Functions:

  • Conducts Home Visits as needed and clinical assessments, Initial Assessment and Re-Assessments.
  • Develops Care Plans that are appropriate to meet the members needs
  • Participates in Clinical Review meetings to determine the appropriateness of care plans
  • Completes and prepares all necessary assessment, data, form and care plans as required.
  • Keeps updated progress notes in accordance with BSHC documentation standards.
  • Works closely with, case managers, consumers, and caregivers to coordinate services.
  • Prepares reports as required.
  • Competencies:

    • Ability to assess individuals to determine clinical eligibility
    • Ability to review assessments/determinations to ensure compliance with program guidelines
    • Knowledge of the biological functions in working with elderly and disabled adults living in a community setting.
    • Considerable knowledge of psychosocial/medical assessment process.
    • Knowledge of the aging process
    • Experience working with the elderly and/or experience working in a community service setting
    • Strong organizational, time-management and priority-setting skills
    • Ability to work with clients in urban, multi-ethnic and multi-racial communities
    • Excellent communication and presentation skills
    • Flexibility to accept changing priorities
    • Basic knowledge of community resources and programs
    • Ability to work independently and as part of a team
    • Comprehensive working knowledge of Microsoft applications including Word, PowerPoint, and Outlook.

    Work Environment:

    This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

    • While performing the duties of this job, the employee must be able to carry a 15 to 20 pound load.

    Position Type/Expected Hours of Work:

    This is a per diem part-time hourly position. Schedule to be determined and assigned by the program manager.

    Travel:

    This position requires 100% travel throughout the Boston community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions.

    Required Education and Experience:

    Licensed Registered Nurse with extensive knowledge of MassHealth Programs, a minimum of four (4) years experience working with elderly preferred.
