Position#Summary Facilitates the implementation, coordination, training and monitoring of system-wide performance improvement efforts. # Primary Position Responsibilities 1. Exhibits expertise in process improvement, team facilitation, coaching and change management 2. Identifies opportunities for clinical and or process improvement and facilitates needed changes to improve patient care, organizational effectiveness and compliance with regulatory standards of care 3. Perform continuous monitoring of process improvement activities to maintain controlled results 4. Performs project management duties for the department#s performance improvement projects 5. Facilitates cooperative and collaborative relationships among departments, physicians and external agencies to ensure effective patient outcomes 6. Acquires, validates, interprets, and transforms data into actionable information 7. Performs Root Causes Analyses and FMEAs developing and providing oversight to action plans # Qualifications Minimum Education: Required: Bachelor#s Degree in a healthcare related field Preferred: Master#s Degree in either Business/Nursing/Healthcare related Minimum Years of Experience (Amount, Type and Variation): Required: Minimum of two- five years in Healthcare Quality/Performance Improvement with a proven track record of developing and implementing projects using quality improvement principles and project management skills. Preferred: Minimum 3-5 years hospital based License, Registry or Certification: Required: None Preferred: Lean Six Sigma Registered Nurse (RN) Current Certified Professional of Healthcare Quality (CPHQ) preferred or obtains certification within 2 years of hire Knowledge, Skills and/or Abilities: Required: Intermediate to advanced proficiency with word processing (Microsoft Word) , database software (Microsoft Excel), and presentation software (Microsoft PowerPoint) required and/or Google Suite Preferred: Lean Six SigmaPosition SummaryFacilitates the implementation, coordination, training and monitoring of system-wide performance improvement efforts.Primary Position Responsibilities1.
