Hourly Range: BR - BR Salary Range: BR,092.80 - BR,342.40 Incentives: BR,000 Sign on Bonus/ 1st Day Benefits. This position qualifies for a BR sign on bonus. Acceptance of this bonus requires a two-year commitment with the bonus paid out in 3 installments. Mercy LIFE is a member of Trinity Health PACE. Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. The Personal Care Assistant is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. This position will primarily involve both home care assignments as well as work in the center. TASKS AND RESPONSIBILITIES:

  • Engage in ongoing participant assessment and monitoring and report on services provided and the self-care abilities of participant.
  • Provide personal care assistance with activities of daily living, including toileting, showers, eating, and grooming.
  • Assist in the maintenance of physical activity, exercise, and walking.
  • Identification of safety hazards and emergency situations and initiating corrective action as able.
  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
  • Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor
  • Escort participants to appointments as necessary.
  • Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PO vehicles as needed.

  • High School Diploma or equivalent Required.
  • Must be an a active certified nurse in the state where PACE organization is located. Required.
  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. Required.
  • Experience in a long-term care facility or with a community-based geriatric program Preferred.
  • Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change.
  • Ability to work with minimal supervision and exercise independent judgment.
  • Superior written and verbal communication and presentation skills appropriate for audience comprehension.
  • Valid drivers license and use of personal vehicle for home care visits
  • Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service.
  • Position may require 10% travel within the Centers geographic region and use of personal vehicle.

On Call Requirements : on call weekend every 7 weeks
