Mercy LIFE Alabama, a member of Trinity Health PACE in Mobile, AL, is seeking an experienced Nurse Practitioner.PACE offers continuous, quality care to the elderly, allowing them the independence of living at home. Job Description:
  • Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today's fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.
  • Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.
  • The primary purpose of this position is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s).
  • Performs assessments in collaboration with the physician and as needed.
  • Develops and implements appropriate plans of care to PO participants.
  • Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education.
  • Functions as a member of the Interdisciplinary Team (IDT).
  • Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant's caregivers, as well as documenting changes in a participant's medical record consistent with documentation policies.
  • Provide primary medical coverage in collaboration with PO physicians as needed. Performs initial, semi-annual and annual assessments, performs additional assessments of participants as needed and implements appropriate plans of care to PO participants.
  • Participates in discharge planning for acute and long-term placement.
  • Evaluate and treat participants during acute illness. Prudently prescribes medications, therapies and other treatments for participants. Following consultation with the Primary Care Physician refers participants to medical specialist as indicated.
  • Make nursing home/or home visits when needed.
  • Work with Medical Director and Clinic Supervisor to formulate clinical policies, procedures and standards of care. Assists with the development of standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
  • Advise the Medical Director and/or Primary Care Provider in ways and means to establish better accountability of the PO services to participants and referral sources. Acts as resource during intake of new participants and in the day-to-day operation of the center programs.
  • Participate in provider on-call rotation as required.
  • Must be action-oriented, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility.
  • Must have a valid driver's license, proof of insurance and have means of transportation
  • Can only accept NPs, will not accept PAs
  • At last 5-7 years of NP experience, preferably in long-term care setting required
  • Minimum of one (1) year working with the frail and/or elderly population required
  • Not accepting new graduates
  • Not accepting Locums candidates

  • ANCC/AANP Board Certified - REQUIRED
  • AL license (or willing to obtain) - REQUIRED
  • 5-7 years of NP experience, preferably in long-term care (note in Experience field) - REQUIRED
  • Minimum of 1 year working with the frail and/or elderly population (note in Experience field) - REQUIRED
  • BLS (or willing to obtain) - REQUIRED
  • CSR/CDS Certification (or willing to obtain) - REQUIRED
  • DEA (or willing to obtain) - REQUIRED
  • Clean/Clear malpractice - HIGHLY PREFERRED
