Permanency Coordinator! Serving as a vital link between students, families, agencies, and community resources, you'll ensure seamless collaboration and unwavering support at every turn. - Cultivating strong relationships through family visits, team meetings, and community engagement, all rooted in our trauma-informed approach.
- Contributing actively to both group and individual programming, nurturing growth across academic, career, and personal/social domains.
- Upholding the values of The Building Bridges and Families First Initiatives by championing permanency within the student's own community, offering unwavering support and coordination.
- Playing a pivotal role in Individualized Transition Plan (ITP) meetings, as well as providing vital support during case plans and court hearings.
- Keeping meticulous records and paperwork, ensuring accuracy and compliance with our standards to uphold our commitment to excellence.

Reporting to our dedicated Clinical Director, you'll receive ongoing supervision and mentoring to help you thrive in your role. Requirements: - A valid driver's license and a minimum age of 21.
- Bachelor's Degree in Human Services, Social Work, or Criminal Justice.
