Assists with toilet needs and ambulation and transfer of the patient/client.Supports the patient/client's nutrition through meal planning, purchasing groceries, preparing and serving food according to dietary needs.Provides household services deemed necessary to maintain a safe, hygienic environment.Performs special procedures when formally trained to do so, under the direction and authority of the supervisor, within scope of practice.Reports pertinent information, including changes in condition/problems to the supervisor.Maintains continuity of care through the support of the home care team.Provides exceptional level of care with compassion and sensitivity to those nearing the end of life's journey.Administers medications as directed by the Medication Administration Record only after certification is obtained.AdditionalDemonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs.
