Personal Care AssistantPOSITION SUMMARY: Performs and assists participants with Activities of Daily Living (ADLs) such as participant personal care and toileting, assistance with meals, laundry, restorative and supportive care. Monitors health status and provides a safe environment for participants. Assists with organization and implementation of activity program, meal service, and other center services as directed. Functions as a member of the interdisciplinary team and communicates with team members regarding participant condition and status changes. EXPERIENCE AND EDUCATION:
  • Two (2) years of experience working in a hospital, nursing home or community-based setting is preferable.
  • License/Certification as applicable per State requirements
  • 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
  • Position specific competencies for the Personal Care Specialist will be met prior to assuming participant care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Deliver high-quality care as a member of the PACE interdisciplinary team (IDT), following each participant's plan of care across various settings, including the center, clinic, contracted facilities, and participants' homes.
  • Provide personal care assistance, including bathing, dressing, grooming, feeding, toileting, and other daily living activities.
  • Facilitate and participate in group and one-on-one activities aligned with the daily activity calendar.
  • Assist with meal preparation, service, monitoring, and cleanup.
  • Maintain a clean, safe, and orderly environment for participants throughout the day.
  • Record and monitor vital signs, weights, and changes in participants' physical or emotional health, reporting to the appropriate team members.
  • Accompany participants to appointments, assist with transportation, and provide escort services ensuring participant safety.
  • Collaborate with the IDT to develop and implement individualized care plans and communicate participant needs effectively.
  • Maintain accurate documentation in participant medical records and meet charting/reporting requirements.
  • Protect privacy and maintain confidentiality of all company procedures, results and information about employees, participants, and families.
  • Foster positive relationships with participants, families, co-workers, and community members.
  • Participate in team meetings, in-services, and training to support quality improvement and ongoing education.
  • Follow organizational policies, procedures, and OSHA safety guidelines, maintaining confidentiality and professional standards.
  • Participate in continuing education classes and any required staff and training meetings.
  • Perform additional duties as assigned to support the care and well-being of participants.
PRE-EMPLOYMENT REQUIREMENTS:
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
SKILLS AND ABILITIES:

  • Knowledge of principles, practices, standards and techniques of a CNA / HHA.
  • Knowledge of common safety hazards and precautions for a safe work environment
  • Knowledge in working with frail and elderly to support Activities of Daily Living and Incidental Activities of Daily Living.
  • Ability to work independently with minimum supervision.
  • Ability to communicate clearly and effectively.
  • Ability to react calmly and effectively in emergency situations
  • Basic computer literacy
  • Ability to work effectively with culturally, economically and educationally diverse populations.
  • Able to manage time well and be dependable.
  • Ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials.
TECHNICAL SKILLS:
  • Computer, keyboard, mouse, copier, scanner, telephone console, calculator, full training with all Windows applications, Micro-soft Office E-mail, Word, Excel and PowerPoint and all related office supplies and equipment. Experience with EMR, Strong computer literacy: Microsoft applications including outlook, word, excel and PowerPoint, ability to correspondence effectively in writing.

  • The employee must effectively engage within an interdisciplinary team model, collaborating with diverse clinical and social services professionals to ensure comprehensive care for PACE participants. The work environment includes an Adult Day Health Center and primary care clinic, characterized by moderate noise levels and controlled temperatures.
