Personal Care LPN Position: 8 hr / wks, 6:30am - 2:30pm; every other weekend (Saturday and Sunday) Personal Care LPN Pay: $29.60 - $32.00 hour, + $1.25 / hr weekend shift differential Summary and Purpose: Core Values: Compassion, Community, Dignity, Integrity and Quality, Teamwork and Trust Create and support a caring and empathetic working environment Know and observe all policies and procedures for Personal Care Follow and support the Fairmount mission vision and values Employment Qualifications: • Currently licensed as a Licensed Practical Nurse with the Commonwealth of Pennsylvania. • Current CPR certification. • Complies with state and federal regulations • Knowledgeable of Scope of Practice • Able to work throughout the facility • Demonstrates critical problem-solving and appropriate independent decision-making abilities. • Communicates professionally with the medical staff, nursing personnel, residents and families. • Follows Fairmount's policies and procedures • Demonstrates good verbal and written communication skills in the English language. • Able to perform a variety of duties, often changing from one task to another of a different nature without loss of efficiency. • Able to communicate with others clearly and concisely and to identify task requirements and monitor progress toward accomplishment • Able to maintain relationships that facilitate task accomplishment, to cooperate and resolve conflicts, and to recognize needs and be sensitive to others • Interacts and works well with others and enjoys working with people • Has a sincere desire to work with older, disabled or handicapped adults • Maintains a positive attitude and actively promotes good communication with all staff on all shifts. • Is tactful, courteous, patient, pleasant and cheerful when dealing with nursing staff, residents, medical staff, families, and/or the general public. Demonstrates a willingness to learn and show dependability and competence. • Supports the mission, philosophy, policies and core values of Fairmount Homes. Essential Duties and Responsibilities:
Accounts for narcotics for the assigned neighborhoods Receives report from previous shift's charge nurse for assigned neighborhoods and gives same to subsequent shift's Charge Nurse. Notifies Resident Assistants of any unusual or additional care needed for residents assigned to them. Administers medications and completes treatments per physician's orders following the Pharmacy Policy and Procedure Manual. Makes rounds, checking all residents. . Completes nursing documentation on resident's records (i.e. progress notes, flow sheets, monthly summaries, etc.). Assigns additional duties to Resident Assistants (i.e. weekly supply orders and PRN, cleaning chores, etc.). Assists with resident assessments, admissions, discharges, transfers and deaths. Receives written, faxed and verbal orders per facility policy. Maintains safety and infection control standards following the regulations of the State and Federal regulatory agencies. Initiates Incident and Accident reports per policy. Keeps Director of Personal Care informed of status of residents including all incidents and accidents. Supervises Resident Assistants and requires accountability to comply with the facilities policies and procedures. Responsible for attending mandatory meetings. Adheres to Corporate Compliance Plan. Complies with HIPAA Privacy standards. Completes additional tasks as required to ensure that the highest quality care is maintained. Physical and Composure Requirements:• VISION: Requires constant overall vision including peripheral vision, color and depth perception. • MOBILITY AND ACTIVITY LEVEL: Extensive standing, full use of both hands, frequent walking, bending, twisting, stooping, kneeling, climbing, reaching, balancing. Must be able to lift up to 50 lbs. frequently. • HEARING/TALKING: Hears, talks and understands normal levels of speech. Mental/Emotional Demand: Must have the ability to function calmly and professionally under stressful conditions. Must be able to communicate clearly with others, in an empathetic manner. Must be able to make others feel comfortable and create trust. Must be able to respond in an effective and caring manner when confronted with trauma, death and grief.
