Position Summary:
The PCT provides direct support in-home services to one or more consumers with intellectual and/or physical disabilities by promoting choice, personal growth, and independence in their homes and community. Overall, the PCT is expected to provide activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
PCT may work under the supervision of the Personal Care Technician-Advanced, and/or the House Manager / Case Manager. Also, the PCT position will be required to work occasional weekend shifts. The specific job duties for the PCT are designed around the needs of each individual consumer and may be modified as the needs of the consumer change.
Essential Functions:
Provide instruction and assistance in daily activities and routines such as:
1. Maintain healthy and professional boundaries with consumer and staff
2. May provide for transportation of consumer
3. Meal planning and preparation
4. Personal care and appearance (e.g. bathing, toileting, transferring, dressing and exercising).
5. Passing medications accurately
6. Maintain a clean and orderly home environment (e.g. assisting consumers in the completion of laundry and household cleaning)
7. Provide problem-solving assistance and companionship
8. Accompany the consumer into the community to engage in recreational activities and doctor appointments
9. Maintain records and documentation in the home
10. Ability to work effectively with team
11. Ability to work independently and remain organized and efficient
12. Participation in the job related training's, house meeting and supervisory meetings as scheduled
13. Assist individuals to develop and expand participation in typical community activities and build relationships with family, friends and neighbors
14. Comply with HIPAA Privacy Act by protecting confidential consumer information
15. All other duties as assigned

Physical Requirements
1. Must have a good mobility (e.g. bend, lift, stand, walk, etc.)
2. Must have the ability to lift-up to 50 pounds
Required Education and Experience
* High school diploma or Equivalent
2. Prefer one year of relevant personal care experience

1. Must be able to satisfy the post offer requirements (i.e. Wisconsin Caregiver Background and medical screenings)
2. Some placements may have required use of a personal or company vehicle to transport consumer as needed. All roles requiring driving require a valid driver license and insurance
