Job Description:POSITION SUMMARYThe Medical Social Worker (MSW) evaluates psychosocial status and needs of patients (clients) and their caregivers/family while focusing on the emotional, social and environmental challenges related to chronic and acute illnesses. Provides individuals, families and caregivers with the psychosocial support needed to cope with chronic, acute and terminal illnesses. Services include advising patients and family caregivers, providing education, referrals to community agencies for additional support and services and patient advocacy in collaboration with the RN Clinic Manager and the Interdisciplinary Team. May also provide care management or interventions designed to promote health, manage chronic diseases and address barriers to healthcare access. Engages in direct services for patients, such as chart review, phone contact and home visits. Supervises, evaluates and documents patient's progress in line with standard goals illustrated in patients identified goals. Ensures quality of services and compliance with regulatory standards.
POSITION QUALIFICATIONSEducation/Licensure/Certification:
- Master's Degree in Social Work required.
- *Must meet DMV requirements listed below and have adequate transportation.
Knowledge:
- Minimum one year of social work experience in a health care setting.
- Demonstrated leadership, integrity, initiative and effective communication skills.
- Knowledge of Advanced Directives.
- Knowledge of community resources and health care financial system.
Skills:
- Experience with team building and collaboration, consensus building and group facilitation.
- Excellent verbal and written communication skills, openness to change and willingness to try new approaches.
- Proficiency in utilizing computer applications and electronic medical records to input and extract data accurately, receive and send emails, as well as in making calendar appointments.
- Unique capability to manage time effectively.
- Must possess the ability to work efficiently with little or no direction or supervision, self-directed and proven effective organization and prioritization skills.
- Willingness to travel according to the standards of the job.
- Must have the physical and mental health adequate to meet the demands of the job.
*Note: Requires employees to drive at least once per pay period. These positions require driving as an Essential Job Function, where an alternative means of transportation, such as ride-share or public transportation is not feasible to complete these duties. This may be averaged over the calendar year. Candidates must possess and supply proof of: a current CA Driver's License, a clean DMV printout and Auto Insurance coverage with a minimum of $50,000 per person/$100,000 per occurrence (or a combined single limit of $100,000) and property damage coverage in an amount not less than $50,000 per occurrence.
