This includes purchasing; distribution; dispensing; administration; packing; storage and security; prospective and retrospective monitoring of patient medication regimens; maintenance of required records; preparation of reports; and maintaining communication with all staff. Duties and Responsibilities:
Develops, implements, reviews and amends Policies and Procedures related to pharmaceutical services in conjunction with hospital administration, the Pharmacy and Therapeutics Committee, and other hospital staff.Develops criteria for departmental quality assurance studies; monitors, documents, and submits related reports to Performance Improvement Committee. Serves as a liaison for the Pharmacy and Therapeutics function of the Medical Staff through the preparation and presentation of formulary requests. Maintains the hospital formulary through the incorporation of additions and editing out deletions as approved by the Medical Staff. Develops and conducts in-services for staff when needed. Maintains perpetual inventories for all controlled substances. Supervises purchases of pharmaceutical products for use within hospital departments. Records and reports adverse drug reactions to the Risk Manager via the Medication Variance Report within 3 days of occurrence and immediately if a serious incident. Records and reports near misses and medication errors to the Risk Manager via the Medication Variance Report within 3 days of occurrence and immediately if a serious incident.Acts as chairman of the hospital's Patient Medication Safety Sub-Committee in order to help improve safety of the drug use process. Additional responsibilities in the full job description.RequirementsEducation: Graduate of Accredited College of PharmacyExperience: 5-7 years as a Pharmacist, Hospital experience highly preferredSkills: Current License as a Registered Pharmacist and a Consultant Pharmacist in the State of Florida.Knowledge: Knowledge of duties of a Hospital Pharmacist. CPOE and ADCAbilities: Ability to perform tasks of a Hospital Pharmacist. Computer proficient in CPOE and ADCPersonnel SupervisedStaff Pharmacist, Pharmacy Technician
