Job Description: UPMC Hamot in Erie, PA is seeking a highly motivated and detail-oriented Medication Reconciliation Pharmacy Ops Specialist, Associate to join our dynamic pharmacy team. This specialist will play a critical role in ensuring accurate medication histories for patients, contributing to improved patient outcomes and safety. Our full-time role will serve 10-hour evening shifts with rotating weekends holidays. Primary work will be focused on admission medication reconciliations for patient admitting through the emergency department.\n Patient Interviews: Obtain detailed medication histories through patient interviews, family members, and outpatient pharmacy records.\n Documentation: Document medication histories accurately in the electronic health record (EHR) system.\n Collaboration: Work closely with pharmacists, physicians, nurses, and other healthcare professionals to clarify discrepancies and ensure accurate medication information.\n Education: Provide education to patients and caregivers regarding their medications.\nQuality Improvement: Participate in quality improvement initiatives related to medication reconciliation processes.\n Compliance: Ensure compliance with Joint Commission standards and hospital policies regarding medication management. \n\n Qualifications:\n Required\n\nCompletion of high school diploma or equivalent require OR \nCompletion of an accredited Pharmacy Technician Program\n\n AND one of the following: \n\nTwo years of previous pharmacy technician experience OR\nEnrollment in good standing as a pharmacy student OR\nDemonstrated competency in all detailed aspects of the Pharmacy Operations Specialists DESIGNATED area which includes (Inventory Control, Chemotherapy, Medication Safety, Automation, Information Technology..etc)\n\n Preferred\n\nPharmacy technician experience in a hospital or retail setting.\n Previous experience with medication reconciliation is highly preferred\nStrong attention to detail and excellent organizational skills\nProficiency in using electronic health records (EHR) systems\nExcellent communication and interpersonal skills\nAbility to work independently and as part of a multidisciplinary team.\n Comprehensive understanding of pharmacology and medication safety principles\nFamiliarity with Joint Commission standards and hospital medication management policies\n\nLicensure, Certifications, and Clearances:N/AUPMC is an Equal Opportunity Employer/Disability/Veteran
