Job Description: UPMC Rx Partners is looking for a full time Pharmacy Patient Care Coordinator to join their dedicated team!\nThe Pharmacy Patient Care Coord. provides care to patients within the UPMC Health System by primarily assisting with data entry tasks and ancillary, supportive and clinical tasks while under the direct supervision of a licensed pharmacist. The Pharmacy Patient Care Coord. services are provided in compliance with Department of Health, ACHC State Board of Pharmacy regulations and other accrediting bodies.\nThis role will work rotating daylight and evening shifts, as well as potential rotating weekend shifts.\nUPMC offers a generous benefits package, an expansive total rewards package, opportunity to grow your career and much more. Apply today!\n\nResponsibilities\n\nProcess refills and patient inquiries in accordance with all governing regulatory agencies including but not limited to ACHC, and state board of pharmacy under the supervision of a pharmacist.\nProvide excellent internal and external customer service to achieve optimal patient satisfaction scores.\nAct as a conduit of information between the pharmacy, operations and clinical staff.\nInterface with other departments and identify documentation to support accreditation standards.\nComplete standard assessments according to accreditation guidelines.\nMaintain or exceed designated quality and production standards of the pharmacy department.\nReport clinical issues and concerns to a pharmacist.\nParticipates in staff meetings that occur 1-2 times a month, as well as any pharmaceutical educational in-services provided.\nCommunicates verbally with patients, caregivers and other staff members to obtain and document patient medication inventory as well as patient assessment data and follow-up for pharmacist review.\nAssists in timely preparation of all pharmacy assessment and documentation including prescription labels, safety assessments and medication profiles.\nEntering of patient data into system, management of electronic prescriptions and prescription refills, attachment of documents to patient's profile, adding and maintaining the prescriber database in the system and adding drug orders/drug setup into the patient's medical record.\nProvide and receive information via telephone, fax, email or written to/from referral sources, personnel, and patients. Refer all drug information questions to pharmacists.\n*Performs in accordance with system-wide competencies/behaviors.\n*Performs other duties as assigned.\n\nEducational and Knowledge Requirements\n\nHS diploma or equivalent required 2 years of pharmacy technician, and/or other related experience in a pharmacy setting required. \nExcellent knowledge in retail pharmacy processes and/or specialty pharmacy functions preferred. \nExcellent knowledge of medical terminology and pharmacy terminology required. \nThorough knowledge in MS Office and PC skills required. \nCustomer Service experience with communication via telephone and e-mail required. \nExperience with prioritizing and multitasking required\n\n\nLicenses and Certifications Notes\n\nPharmacy Technician certification is desired\n
