Concord Hospital is seeking a Physical Therapist to join our team. It is an exciting opportunity to work in an outpatient rehabilitation clinic with a strong orthopedic and manual therapy program. Our patient population is diverse and includes patients with both orthopedic and neurological deficits as well as sports related injuries. We also treat many specialty diagnoses including men's/women's health, vestibular, lymphedema, oncology, concussion and LSVT Big. We provide one on one individualized patient care in 45 minute appointments for both initial appointments and follow up visits. Staff development and engagement is a strong focus and we have an excellent mentorship program.

Minimum Starting Pay: $74,874.98
Summary
The Physical Therapist I Outpatient provides patient evaluation and treatment according to established policies and procedures for their discipline to patients of all ages and populations.
Education
The physical therapist must have graduated from an accredited Physical Therapy program.
Certification, Registration & Licensure
Certification required within 90 days of hire and re-certification within 30 days of expiration: Current American Heart Association Basic Life Support for Healthcare Providers certification, or equivalent course credentialed by American Heart Association (American Red Cross CPR/AED for Professional Rescuers and Health Care Providers).
Licensure Required: Current license to practice as a Physical Therapist in the State of NH
Experience
Requires experience working with a multidisciplinary team, patients and others. Must possess knowledge of growth and development across a diverse age and multicultural population.
Responsibilities
* Performs patient evaluations, develops and implements treatment plans in accordance with licensure, scope of practice and department standards.
* Re-evaluates patients according to department standards. Develops, re-assesses and updates goals for all patient episodes of care.
* Demonstrates knowledge and understanding of the profound and prolonged impact of disability upon an individuals' physical, emotional, social, economic and vocational status.
* Documentation is compliant with HIMS/QA standards and meets all customer and medico legal needs.
* Maintains a level of productivity that supports patient access and clinic needs
* Manages all insurance and charging requirements.
* Advocates for and is active in developing team members.
* Takes responsibility for individual performance goals
* Completes hospital and departmental requirements according to established departmental procedures
including (though not limited to): timely completion of documentation and discharges; submitting payroll and completing Work Day requirements on time; reviewing meeting minutes and
taking the initiative to obtain additional information as needed; completing all clinical competencies;
completing all aspects of the self review portion of the performance review process; contributing
meaningful, objective and constructive peer feedback for performance reviews, completing all hospital web compliance assignments; insuring CPR certification is active.
* Participates in hospital and/or departmental initiatives/special projects, including though not limited to:
student supervisor; job shadow/resident shadow; community education; program development;
professional promotional activities; presenting in-services or projects related to coursework; hospital wide projects or initiatives.
