$9,000 Sign On BonusPOSITION SUMMARY:The Physical Therapist provides skilled physical therapy services consistent with professional and regulatory standards and policies. Participates in the development of individualized programs necessary to achieve patient care objectives.ESSENTIAL DUTIES AND RESPONSIBILITIESPerform comprehensive patient assessments to determine functional deficit and potential and establish a therapeutic program. Develop a therapy plan of care to include goals and discharge plans.Document, within time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition. Completes all documentation required and in accordance with state and federal regulations and policy. Demonstrate administration of objective, standardized tests and measures and performance of evidence-based practice in documentation.Communicate and coordinate patient care with other members of the patient's health care team. Attend employee meetings and patient conferences.Supervise physical therapy assistants in accordance with regulations and policy.Provide patient and family teaching related to the patient's physical therapy program and patient/family engagement in goal achievement. Assist the patient and/or family to procure needed equipment or supplies.Provide physical therapy in interventions accordance with physician's orders and accepted evidence-based professional practices.Utilize appropriate hand washing, standard precautions and personal protective equipment.Keep knowledge of physical therapy theory and practice current and acquires new skills as needed by attendance at workshops, courses, in-services, etc.Demonstrates proficiency with the administration of objective standardized tests and measures and compliance with regulatory standards.POSITION REQUIREMENTS:Graduate of an accredited Physical Therapy programLicensed as a PT in the MassachusettsHome care experience, strongly preferredCurrent CPR requiredCurrent State of Residence driver's license and reliable transportationExcellent organizational and interpersonal skillsComputer proficiencyExcellent verbal and written communication skillsAbility to read, write and speak the English languageMaintain required vaccinationsPHYSICAL REQUIREMENTS/WORKING CONDITIONS:Work primarily indoorsPossible exposure to unpleasant odorsPossible exposure to chemicals as identified in the MSDS ManualPossible exposure to infections, diseases and Bloodborne pathogensContinuous exposure to patients who are ill, confused, irritable and irrationalSubject to hostile and emotionally upset family members, personnel and visitorsMay work beyond normal working hours and on weekends and holidays when necessary.IND3
