We value the quality of service provided to each patient.*The Position: ALF; Physical Therapist*The Physical Therapist provides physician-prescribed Physical therapy to improve or minimize residual physical disabilities of the patient. Maintains ongoing communication with Physicians, healthcare team, and other individuals as needed regarding aspects of care and patient s status. Develop the plan of care and revise it as necessary. Communicates with PTPlus pertaining to matters related to Rehab services and indicated patient care.*Position Qualifications:* ONE (1) year of experience, preferred. Licensed by The State of New York as a Physical Therapist. Graduate from a physical therapist curriculum approved by one of the following: The Commission on Accreditation in Physical Therapy Education (CAPTE) or successor organizations of CAPTE.
