Physical Therapy Assistant (PTA) at Jerome Home Jerome Home has an immediate position Per Diem Physical Therapy Assistant to join our team for short-term rehab and caring for our long-term residents. The position offers competitive salary and flexible schedule. Jerome Home is a 120 bed Long Term Care Facility in New Britain, CT. Established under the will executed by Augustus Jerome, Jerome Home is a privately-owned, not-for-profit organization. We are a progressive nursing center with a stable, highly competent and caring staff. Established in 1932, our multi levels include, RCH and SNF including short-term rehab and hospice. We are using Rehab Optima and Matrix programs. We have several on-line programs currentlybeing used by our staff:
  • Matrix is used as our EMR
  • Smartforce is our on-line scheduling program. At the click of a button, you can request time off or request to pick up a shift. You can also send email/text notification to request coverage of an open shift
  • Relias is our on-line education program. Classes are assigned via email for you to complete in the allotted time at your own pace
  • Paylocity is our online payroll system. We are all direct deposit and you are able to view your check right online. In addition, we communicate to staff through this system
The primary purpose of your job position is to:
  • Assist the Physical Therapist to plan, organize, develop and direct services in accordance with current applicable federal, state and local standards, guidelines and regulations, and as may be directed by the Rehab Program Manager to assure that the highest degree of quality resident care can be maintained at all times
  • Interpret physical therapy policies and procedures to staff, residents, family members, etc., as necessary
  • Participate in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary
  • Review request for physical therapy and physicians' orders
  • Interview resident to determine type of treatment and schedule therapy
The successful candidate
  • Must possess a valid PTA License from an accredited college or university
  • Must have experience as a PTA in a hospital, SNF or other related medical facility
  • Must have ability to work with our old population
