Physical Therapist Assistant, Full Time
Polaris Health Care and Rehabilitation Center is looking for an Physical Therapist Assistant to join our rehabilitation team and provide therapy services to patients in our skilled nursing facility. The ideal candidate will be passionate about their patients and committed to helping them recover their independence. We are looking for someone with experience working in a skilled nursing facility . The right candidate is patient, compassionate and professional who encourages and inspires their patients to work toward greater independence and recovery.
Responsibilities for Physical Therapist Assistant
- Implement treatment plans established by the physical therapist to restore function and performance
- Provide dynamic, creative, and challenging treatments to patients to facilitate progress/improvement
- Monitor patients' performance in therapy activities and adjust treatment program to obtain improved progress
- Collaborate with the interdisciplinary therapy team members to provide input regarding patient needs and establish a rehabilitation plan.
- Maintain positive interaction with patients, families and caregivers while providing instruction and education for the skills and functions essential for promoting patient independence in collaboration with the supervising physical therapist
Qualifications for Physical Therapist Assistant
- Experience providing physical therapy to patients in a skilled nursing facility
- Graduated from an accredited physical therapy assistant program
- Current Physical Therapy Assistant license for the state of Delaware
- Current CPR certificate
- Must have good organizational skills, time management skills and good verbal/written communication skills
