Job DescriptionFT Physical Therapist positions available at an Inpatient Rehabilitation Facility (IRF) in Hummelstown, PA.CONTRACT DETAILS:
- $2,300/wk
- 13 contract with possibility to extend
- GUARANTEED 40 hour work week (OT paid after 48)
- Shift: 5x8 - Days
- Every other weekend required: Yes
SCOPE OF WORK:
- Patient Evaluation and Assessment: conduct thorough assessments of patients' physical abilities, limitations, and functional goals. Evaluate strength, range of motion, coordination, balance, and other relevant factors to develop appropriate treatment plans.
- Treatment Planning and Implementation: Based on the assessment, develop and modify personalized treatment plans and goals that incorporate therapeutic exercises, manual therapy techniques, modalities, and functional training.
- Collaboration: Collaborate with other healthcare professionals, such as physicians, nurses, and occupational therapists, to coordinate patient care. This includes timely equipment trialing and ordering, issuing of a home exercise program, and planning for continuing services.
- Rehabilitation and Intervention: provide hands-on interventions, prescribe therapeutic exercises, and use specialized equipment to facilitate healing and recovery. Focus on improving muscle strength, flexibility, joint mobility, coordination, balance, and overall physical performance.
- Patient Education and Home Exercise Programs: Educate patients and their families about their condition, treatment options, and strategies for self-management. Develop home exercise programs and provide guidance on injury prevention, ergonomics, and lifestyle modifications to promote long-term well-being.
- Documentation and Charting: clearly and concisely document patient progress, treatment plans, and outcomes to ensure accurate communication and continuity of care in a timely manner. Ensure care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care of the patient.
QUALIFICATIONS:
- Current PA state licensure as a Physical Therapist required.
- Certified BLS required.
