SUMMARY:# Responsible and accountable for completing a physical therapy evaluation for each referred patient and for creating and implementing a physical therapy care plan based on the completed evaluation to restore a patient#s function, alleviate pain, and prevent disability. # Baxter Health COMPLIANCE RESPONSIBILITIES Understands and adheres to Baxter Health#s#standards as they appear in Baxter Health#s intranet Lippincott/VNAA Library. Required to work with the Director of Home Health, regularly to carryout, initiate and/or implement best practices for the BRMC Home Health Department. As part of its commitment to ethical and legal conduct, the hospital expects all BRMC employees to bring to the attention of the department head, employee hotline (888-###-####), or compliance officer, information regarding any suspected improper conduct. # JOB REQUIREMENTS Education: Graduate of and degree holder from a Physical Therapy Education Program accredited by the Commission on Accreditation in Physical Therapy Education Preferred experience: 12 to 18 months related experience, excluding clinical affiliations required to complete a degree Licensure:# Current Arkansas PT license in good standing required.# Valid and clean Driver#s License, reliable transportation and agency required liability insurance to complete home visits.# Active personal automobile liability insurance coverage consisting of $100,000 per person and $300,000 per accident for bodily injury required.# # Certification:# Current CPR certification Preferred Education: Master#s or DPT Degree Other:# Ability to read, write and communicate effectively.# Basic computer knowledge required.# Driving required. # Primary Source Verification: Original Transcripts and Arkansas State Board of Physical Therapy # Safety Sensitive Designation:# This position is deemed safety sensitive in accordance with Arkansas Act 593 governing Medical Marijuana. # Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job with or without an accommodation.# Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.# While performing the duties of this job, the employee is required to perform the following:# Must have the ability to: communicate effectively, orally and in writing; focus with attention to detail, solve problems and make decisions; show compassion to those in pain/functional loss; handle critical and highly stressful situations with efficiency and composure. Must be able to stand and walk, and bend, squat and twist, push, pull and occasionally lift up to 250 pounds with proper body mechanics for up to 8 hours each day. Requires repetitive hand movements. Must have a keen sense of hearing and visual acuity. Must have good manual dexterity. Must be able to operate a computer keyboard and PC mouse to utilize computer software for patient care documentation. # # # Work Environment: Patient care setting: Home environment There is potential risk for exposure to environmental issues. # Position Type and Expected Hours of Work This is a position in a home setting which is open 24 hours a day, 365 days a year. Direct patient care staff: Monday # Friday 8 hour shifts. Additionally, holiday and weekend work will be scheduled per the current rotation templates.
