- Conduct initial and ongoing comprehensive patient assessments, including OASIS evaluations.
- Collaborate with an interdisciplinary team to develop and implement personalized care plans.
- Assist physicians in patient evaluations through muscle, nerve, and joint tests.
- Educate and supervise patients and their families on therapeutic programs, as well as the use of wheelchairs, braces, crutches, and other mobility aids.
- Provide and coordinate necessary equipment to support patient care plans.
- Document clinical/progress notes within 48 hours of visits and communicate with physicians at least every 30 days or when changes occur.
- Supervise Physical Therapy Assistants and home attendants, ensuring compliance with agency policies.
- Participate in case conferences, team meetings, and in-service training programs.
- Assist in discharge planning and complete Physical Therapy Discharge Summaries.
Qualifications:
- Successful completion of an accredited Physical Therapist education program.
- Valid state licensure as a Registered Physical Therapist.
- Strong communication skills and ability to work independently.
- Experience in home health care is preferred but not required.
