**Service Area:**
You'll primarily cover some will be Maywood, Oak Park, Forest Park, Elmwood Park, Carol Stream and Chicago zip codes north of north aveneue- but other areas around too-ensuring our patients receive top-quality care where they need it most.

**What You Need to Succeed:**
+ Bachelor's or Master's degree in Physical Therapy.
+ Licensed to practice in Illinois.
+ Home healthcare experience and knowledge of participation conditions preferred.
+ Must have a valid driver's license and reliable transportation.
