Physical Therapist - PT - Minimum 1 years as PT required
- Minimum 6 months Hospice / Home Health experience preferred
- Charting system is HCHB (Homecare Homebase)
- On call rotations determined by hospice / home health agency
- To follow local governing board scope of practice.
