Job Description:

Responsibilities: Provides direct restorative treatment of patients, carries out administrative duties associated with patient care, and contributes to department operations appropriate to the program. WORK PERFORMED: 1. Responsible for direct patient/resident care as appropriate and as defined by position level: a. Performs standardized evaluations of patients/residents using standard techniques b. Develops appropriate long and short term goals c. Develops basic treatment plans based on evaluation results that leads to achievement of goals d. Provides age appropriate treatment based on evaluation results that leads to achievement of goals. e. Evaluates and recommends patient/resident need for durable medical equipment, distributes information to patient/resident on purchase or arranges for order and distribution of apparatus, familiar with insurance requirements. f. Completes documentation according to facility time frames and requirements g. Correctly identifies and bills for therapy services rendered. h. Attends and reports at rounds and team conferences i. Aware of safety precautions and able to carry out emergency procedures j. Attends and participates in in-service programs k. Provides patient/resident/family education and training and participates in family conferences l. Demonstrates ability to lead a treatment group m. Communicates with patients/residents clearly and adjusts approach based on their needs/abilities. n. Responsible for 100% direct patient care at assigned site. 2. Contributes to efficient hospital/facility, department and team functioning: a. Active participant in hospital/facility, department and team problem solving and projects b. Volunteers assistance where needed c. Covers for team leader as indicated d. Supervises PTAs, aides and volunteers as indicated e. Notifies the Director/Manager of need for equipment, materials or supplies, or follows department procedure f. Participates in department maintenance g. Maintains an awareness of program/hospital issues, projects a cooperative attitude and promotes problem solving h. Complies with program and facility mission, values and policies i. Provides in-service training for staff as requested. j. Maintains and contributes to safe work environment. k. Participates in new staff orientation/competency assessments 3. Participates in student training program as needed. 4. Actively participates in the supervision process: a. Reviews evaluation results, treatment plan and goals with PTA b. Communicates needs to appropriate program personnel. c. Notifies appropriate personnel of unusual patient/resident findings in a timely manner d. Demonstrates an awareness of limitations and seeks supervision appropriately e. Modifies performance in response to feedback 5. Demonstrates the ability to translate knowledge of physical, psychosocial and cognitive-perceptual changes related to age into assessments, treatment plans and treatment modalities for pediatric, adolescent, young adult, adult and geriatric patients/residents. 6. Completes annual career ladder requirements. 7. Practices Standard Precautions. Follows regulations, policies and guidelines on the prevention of infections; does not eat or drink in patient care areas. 8. Maintains confidentiality of information; abides by HIPAA regulations. 9. Maintains a professional image and follows Hospital's policy on uniforms; consistently wears hospital ID. 10. Demonstrates reliable, on-time attendance, which is an essential function of all jobs at Burke. 11. Completes mandatory annual learning (Learning Harbor) within deadline. 12. Personally adheres to Burke C.A.R.E.S. philosophy by demonstrating compassion, accountability, respect and exceeding expectations through service excellence. 13. Performs other job related duties as assigned. Requirements: B.S., Masters or Doctorate in Physical Therapy. NYS licensed or eligible. EPIC experience preferred. BLS certification required through American Heart Association. Sign on Bonus: Up to $20,000 bonus * Actual offer will depend upon a number of factors including experience, education, certification(s), knowledge and skills
