Position Summary The Physical Therapist (PT) isresponsible for developing and implementing a physical therapy program to treatthe patient's medical diagnosis(s) and/or for functional maintenancerestoration affected by the patient's illness or injury. The PT will developand update, as often as indicated, an individualized plan of care including,but not limited to the patient's functional levels, goals, interventions, andrehabilitation potential.
Education - Graduate of an approved school of Physical Therapy(PT)
Licenses/Certifications - Current state Physical Therapy license
- Must meet initial and ongoing educationalrequirements as stipulated by the state to maintain an active professionallicense
- Current CPR card, valid driver's license,reliable transportation, automobile insurance
