Physical Therapist STARTING RATE: $33.00/hr. ($33.00 - $37.00/hr., dependent upon experience)SHIFT: Per Diem 8am - 4pm BENEFITS:Weekly PaychecksReferral BonusesPeer MentorshipJob Summary: The Physical Therapist will evaluate all patients/residents admitted to the facility and will recommend, in writing, to the attending physician a rehabilitation program if indicated. Upon receipt of the physicians' orders, the Physical Therapist will be responsible for the interpretation and implementation of service and programs concerning physical therapy. The physical therapist will, on a regular basis, review and evaluate all patients/residents, be responsible for all documentation of patients/residents' care plans. Attends and actively participates in all staff and committee meetings related to improving patient /resident care and maintaining the physical therapy department. Job Performance Responsibilities: Evaluates all patients/residents admitted to the facility within 24 hoursRecommends in writing to the attending physician a rehabilitation program, if indicated, in their professional's judgement, to include but not be limited to restorative nursingUpon receipt of physicians oders interpret and implement those services and programs related to physical therapy.
